Medical Forms

I have worked with Premier Radiology since 1997. I now work with them on a freelance basis designing whatever print material, retouching, designs and illustrations they need. This is a small sample of the many forms I have created.

This project was for a website header. The doctors were shot individually and I then outlined them, retouched and moved them into different positions until everyone was happy.
